How to schedule an appointment

Call us at 940-382-8000! State that you would like to schedule an appointment and if you are a new or existing patient. If you are a new patient, we will ask for your full name, date of birth, address, and phone number. Please tell us what days out of the week work for you and your preference for either morning or afternoon. If you are an existing patient we will ask for your full name and date of birth. What should you bring to your appointment?

A patient should bring the following:

• Insurance(s) (if they have insurance)

• State ID or Driver’s License

• Lenses and/or contacts that are used currently

• Medical history (optional)

• Patient paperwork (only if you download it and fill it out at home prior to your appointment)

How early should you get to your appointment?

It is highly recommended to come in 15 to 30 minutes before your scheduled appointment. You will have a 15-minute grace period until you are considered a no-show patient unless you call ahead of time to let us know that you will be late.

Our easy-to-find office is located at 2210 San Jacinto Blvd, Suite 1, Denton, TX 76205, which is off of S. Interstate 35. We are across from Golden Triangle Mall and are in-between Staybridge Suites and Inwood National Bank.
